IVF (Institutional Verification Form)

The pdf document can be completed on your computer but
must be printed out
signed by the student
signed by parent/guardian and returned to the Finacial Aid Office with any other requested documentation.
Completed verification forms should be submitted to the Financial Aid Office - Room 061 in the Ben Parker Student Center during regular business hours: Monday through Friday, 8 am to 5 pm.
